IT ONLY TAKES ONE PARD TO MAKE A DIFFERENCE.

We are a community and we care about each other. One person stepping up to help another can make a world of difference, and as members of a common community anyone can be the one to make that difference for a fellow Pard. If you see something you are concerned about, the Lafayette community needs you to intervene and/or report. Resources to the right provide information about how to talk to someone about whom you are concerned, as well as what Lafayette can do to help.

The following forms can be submitted by any member of the Lafayette or broader community (students, faculty, staff, parents, alumni, etc.).

Reporting an Incident of Concern

Academic-Related Concerns

This form is for students, faculty, or staff of Lafayette College to submit a report to the Academic Integrity Officer regarding any concerns they may have related to alleged academic integrity concerns. Completed forms are routed to the Associate Dean of Advising and Co-Curricular Programs.

Alleged Violations of the Code of Conduct

This form is for students, faculty, staff, and community members of Lafayette College to submit a report to the Student Conduct Officer or the Dean of Students regarding any concerns they may have related to alleged campus policy violations regarding individual students. Completed forms are routed to the Director of Student Conduct & Restorative Practices and the Dean of Students.

Athletic Concerns

This form is for students, faculty, staff, and community members of Lafayette College to submit a report regarding any concerns they may have related to alleged concerns regarding athletes, athletic teams, or other concerns associated with athletics at Lafayette College.  Completed forms are routed to the Associate Director of Compliance & Student Athlete Development and the Director of Athletics.

Bias, Harassment, Discrimination, and/or Retaliation Concerns

This form is for students, faculty, staff, and community members of Lafayette College to submit a report regarding any concerns they may have related to alleged bias, harassment, discrimination, or retaliation concerns. Completed forms are routed to the Director of Institutional Equity.

Group or Organization Behavior, including Hazing

This form is for students, faculty, staff, and community members of Lafayette College to submit a report to the Student Conduct Officer or the Dean of Students regarding any concerns they may have related to alleged campus policy violations regarding student organizations or groups. Reports of Hazing should use this form. Completed forms are routed to the Director of Student Conduct & Restorative Practices and the Dean of Students.

Mental Health, Student of Concerns, Substance Misuse, or Student Support

This form is for students, faculty, staff, and community members of Lafayette College to submit a report regarding any concerns they may have related to student of concerns, mental health, substance misuse, or general request for student support. Completed forms are routed to the Case Manager & Care Coordinator, Dean of Students, and Student Support & Intervention Team (SSIT).

Completed forms are routed to the Director of Institutional Equity/Title IX Coordinator and Deputy Title IX Coordinators.
